Why SAFe Fails

The most common failure mode is not
technical - it is cultural.

Most SAFe implementations produce more ceremonies and fewer results. The organization goes through the motions of PI Planning, stand-ups, and retrospectives, but the underlying operating model, decision-making culture, and funding structure do not change. Agile becomes a layer on top of waterfall rather than a replacement for it.

SAFe fails at the top before it fails at the team level. Executive sponsors who do not change how they govern are the most common cause of scaled agile stalls.

Ceremony without culture change

PI Planning events, ARTs, and retrospectives are adopted as rituals without the leadership behaviors and governance changes that make them work. The result is expensive theater that frustrates both business and delivery teams.
